The patient's most critical lab value is the severely elevated triglyceride (TG) level of 640 mg/dL. TGs > 500 mg/dL significantly increase the risk of acute pancreatitis, making TG reduction the primary therapeutic priority. Fibrates are the most effective class of drugs for lowering very high triglycerides. The patient is already on a high-intensity statin. While combining a statin and a fibrate increases the risk of myopathy, fenofibrate has a much lower risk of this interaction compared to gemfibrozil. Therefore, initiating fenofibrate is the most appropriate and safest choice to address the immediate risk of pancreatitis.

A. Increasing atorvastatin to 80mg would provide only a modest additional TG-lowering effect and is insufficient to manage the severe hypertriglyceridemia and pancreatitis risk.

B. Niacin is not recommended as it can worsen hyperglycemia, which is a concern in this patient with an A1c of 8.1%, and it lacks proven cardiovascular benefit when added to a statin.

D. Gemfibrozil is contraindicated with statins. It inhibits statin metabolism, markedly increasing the risk of severe myopathy and rhabdomyolysis.

E. The proposed dose of fish oil (1 gram/day) is a nutritional supplement dose, not a therapeutic dose. Prescription doses of 2-4 grams/day are needed for significant TG reduction.
